Before completing and submitting an online Deed Poll application form, it’s important to understand your responsibility in accordance with our terms and conditions.
As our Deed Pools are drafted by our automated drafting software using the information you provide on the application form and exactly as you provide it, you are responsible for:
Reading, understanding and agreeing to our Terms & Conditions.
Entering only true and correct information
Entering the information with the correct spelling
Entering the correct punctuation such as commas (,), hyphens (-), Capital Letters (A) and Lower-case letter (a) as and where required such as but not limited to; the start of your name i.e. ‘Daniel,’ and within your Address i.e. 258 Wellington Street.
Ensuring you review the information you provide, edit if necessary to ensure correctness, and only submit the application if you are satisfied that the information you have provided is correct and are satisfied for the information you have provided and exactly hoe you provide it to be drafted onto your Deed Poll and any accompanying documentation.
Once you submit your application and receive your Deed Poll, will not issue any refunds in accordance with the Consumer Rights Act regarding Digital products and our Terms and Conditions.
If you receive your Deed Poll and identify a mistake in your personal details, this mistake is presumed to be due to an error caused by you inserting your personal information and we will not provide a refund or replacement Deed Poll*.
*Unless you have purchased our ‘Premium Deed Poll Option,’ in which case, you have 30 days to notify us of any mistakes on your Deed Poll, and we will issue a free replacement.
If you are applying for a New Adult Deed Poll
Before starting an online Deed Poll application for a New Adult Deed Poll, you will need:
Your Date of Birth
Your Current Full Name (First, Middle and Last)
The New Full Name you wish to adopt (First, Middle and Last).
Your Full Address. The address on your Driving Licence!
What Date you want your name to change. Must be a future date and the same date your witness signs your Deed Poll!
Your Email Address. So we can email you your Deed Poll!
Your Mobile Telephone Number. So we can send you the password to access the password protected Deed Poll document.
To know what Deed Poll package you require. Basic - £19.99, Standard - £39.99 or Premium - £59.99!
Your Debit or Credit Card details. To pay our admin fee!

If you are applying for a New Child Deed Poll
Before starting an online Deed Poll application for a New Child Deed Poll, you will need:
Your Date of Birth
Your Full Name (First, Middle and Last)
You Full Address. The address on your Driving Licence!
Consent to change the child’s name from any other persons holding legal parental responsibility.
The Full Name of any other person holding legal parental responsibility for the child whose name you wish to change.
The Full Address of any other person holding legal parental responsibility for the child whose name you wish to change.
The Child’s Date of Birth
The Child’s Current Full Name (First, Middle and Last)
The Child’s New Full Name you wish to adopt (First, Middle and Last).
The Date you want the child’s name to change. Must be a future date and the same date your witness signs your Deed Poll!
Your Email Address. So we can email you your Deed Poll!

The date your name previously changed.

Before starting an online Deed Poll application for a Replacement Child Deed Poll, you will need:

ACCESSING OUR ONLINE DEED POLL APPLICATION FORM:
Access our online Deed Poll application form by clicking on an 'APPLY NOW' button located throughout our website: www.deedpollagency.uk or by CLICKING HERE!
APPLICATION START PAGE:
The first page you come to on our online deed poll application form will be the introduction page as pictured below:
Simply click ‘Start Application’ to begin your name change journey.
NEW OR REPLACEMENT OPTION PAGE:
When you’ve clicked on the ‘Start Application’ button, you’ll then be taken to the ‘new or Replacement’ option page, where you will be asked 'Do you need a New or Replacement Deed Poll?,' As pictured below:
Simply select ‘New’ for a new Deed Poll document, and then click 'Next.'
*Please Note:
Replacement is for individuals who have had a Deed Poll in the past and require a retrospective Deed Poll to evidence their previous name change!
ADULT OR CHILD OPTION PAGE:
When you’ve selected ‘New’ and then clicked ‘Next,’ you’ll then be taken to the ‘Adult or Child’ options page, where you will be asked the question: ‘Do you need a New Adult or Child Deed Poll?’ As pictured below:
Simply select ‘Adult’ for a new Adult Deed Poll document, and then click 'Next.'
*Please Note:
Child is for persons holding legal parental responsibility who want to change the name of the child they hold legal parental responsibility for, and who is 15 years of age or under, and all persons holding legal parental responsibility consent to the child’s name being changed!
DATE OF BIRTH ENTRY PAGE:
When you’ve selected ‘Adult’ and then clicked ‘Next,’ you’ll then be taken to the ‘Date of Birth’ page, where you will be asked the question: ‘What is your Date of Birth?’ As pictured below:
Simply enter your Date of Birth, and then click 'Next.'
*Example:
Date of Birth: 01/01/2001
*Please Note:
You can choose your Date of Birth from the calendar selector or, you can enter it in the following format: 01/01/2001 – You must be 16 years of age or over to complete our online deed poll application form!
CURRENT FULL NAME ENTRY PAGE:
When you’ve entered your Date of Birth and then clicked ‘Next,’ you’ll then be taken to the ‘Current Full Name page, where you will be asked the question: ‘What is your Current Full Name?’ As pictured below:
Simply enter your Current Full Name, including any Middle Names, Hyphens and using the correct capitalisation and punctuation if and where required. Once that’s done and you’ve checked your entries for correctness, click 'Next.'
*Example:
Capitalisation: Daniel Thomas McBride-Johnson
Lowercase: Daniel Thomas McBride-Johnson
Hyphens: Daniel Thomas McBride-Johnson

It is therefore important to ensure you enter your details correctly and check the correctness of your entries before submitting the online Deed Poll application form, as errors may invalidate your Deed Poll, meaning your name will not legally change. Under the Consumer Rights Act for digital products, we will not issue refunds for errors made on application forms and we will not issue replacement Deed Polls unless you have purchased the ‘Premium’ Deed Poll option. NEW FULL NAME ENTRY PAGE:
When you’ve entered your Current Full Name and then clicked ‘Next,’ you’ll then be taken to the ‘New Full Name page, where you will be asked the question: ‘What will your New Full Name be?’ As pictured below:
Simply enter your New Full Name, including any Middle Names, Hyphens and using the correct capitalisation and punctuation if and where required. Once that’s done and you’ve checked your entries for correctness, click 'Next.'

Simply enter your Current Full Address (the address on your driving licence), and using the correct spelling, capitalisation and punctuation if and where required. Once that’s done and you’ve checked your entries for correctness, click 'Next.'

When you’ve entered your Current Full Address and then clicked ‘Next,’ you’ll then be taken to the ‘Name Change Date page, where you will be asked the question: ‘What Date do you want to legally change your name?’ As pictured below:
Simply enter the Date you want your Deed Poll to show as the Date your Name legally changed.
*Example:
Date: 01/01/2031
*Please Note:
You can choose your name change Date from the calendar selector or, you can enter it in the following format: 01/01/2001 – Your name change Date must be a future Date and the Date you and your chosen witness will meet to sign your Deed Poll.
EMAIL ADDRESS ENTRY PAGE:
When you’ve entered the Date you want your name to legally change and then clicked ‘Next,’ you’ll then be taken to the ‘Email Address page, where you will be asked the question: ‘What is your Email Address?’ As pictured below:
Simply enter and confirm the Email Address you want your Deed Poll to be emailed to.
*Please Note:
Your Deed Poll will be emailed to the Email Address you enter into this section and submit on the application form. As your Deed Poll and any accompanying documentation contains personal and sensitive information, it critically important that you enter the correct Email Address and an Email Address only you have access to. We will not be held responsible for Deed Polls that are sent to incorrect email address or data breaches due to the applicant entering the wrong email address!
We will never email you after we send you your Deed Poll.
MOBILE TELEPHONE NUMBER ENTRY PAGE:
When you’ve entered your Email Address and then clicked ‘Next,’ you’ll then be taken to the ‘Mobile Telephone Number page, where you will be asked the question: ‘What is your Mobile Telephone Number?’ As pictured below:
Simply enter the Mobile Telephone Number you want the password to access the PDF file containing your Deed Poll sent to.
*Example:
Correct Entry: +447123456789 or +4407123456789
Incorrect Entry: 07123456789 or 7123456789
Your Mobile Telephone Number must contain the country code first i.e. +44 for the UK. Please look at the picture above for an example of how to correctly enter your Mobile Telephone Number.
*Please Note:
Your Deed Poll access password will be sent via SMS text message to the Mobile Telephone Number you provide however, your password will also be displayed on the Application Submission page when you submit your application form. IT’S IMPORTANT TO MAKE A NOTE OF THIS PASSWORD.
We will never call or text this Mobile Telephone number after we send you your Deed Poll access password.

When you’ve entered your Mobile Telephone Number and then clicked ‘Next,’ you’ll then be taken to the ‘Deed Poll Package Options page, where you will be asked the question: ‘What Deed Poll Package do you need?’ As pictured below:
Simply select the Deed Poll Package which best suits your needs.

CARD DETAILS ENTRY PAGE:
When you’ve selected your Deed Poll Package and then clicked ‘Next,’ you’ll then be taken to the ‘Card Details page, where you will be required to enter your Debit or Credit Card details. As pictured below:
Simply enter your Debit or Credit card details to make the required payment and click ‘Next.’

REVIEW, EDIT AND SUBMIT PAGE:
When you’ve read, understood and accepted our Terms & Conditions and then clicked ‘Next,’ you’ll then be taken to the ‘Review, Edit and Submit page, where you will be required to review your entries, edit if they are incorrect in both spelling or punctuation. As pictured below:
Simply review your entries for correctness including spelling and punctuation, edit if required. When you are satisfied that your details are correct and contain the correct spelling and punctuation, click the ‘Submit’ button.
*Please Note:
Your Deed Poll and accompanying documentation will contain the information you enter and exactly how you see it displayed on this, the Review, Edit and Submit page. Errors may invalidate your Deed Poll, and no refunds will be issued once the application has been submitted.
APPLICATION SUBMISSION CONFIRMATION PAGE:
When you’ve clicked ‘Submit,’ you’ll then be redirected to the ‘Application Submission page.’ As pictured below:
This page confirms that your application has been received, and our system is working of drafting your Deed Poll and any accompanying documentation.
You will receive your Deed Poll and any accompanying documentation by email, to the email address you have provide within one (1) hour of submitting your application, PLEASE CHECK YOUR SPAM AND JUNK FOLDER.
The Application Submission page will also display your Deed Poll access password, PLEASE MAKE A NOTE OF THIS PASSWORD as you will need it to access the password protected PDF of your Deed Poll.
At the bottom of the submission page will also be a downloadable document link for a Deed Poll Instructions Handbook. This handbook containing instructions of everything you need to do when you receive your Deed Poll.
WHAT DO I DO WHEN MY DEED POLL ARRIVES?
When your Deed Poll arrives by email you must:
Check the details drafted on it are correct
Print as many copies of your Deed Poll as you need to send to your record holders. We recommend 10 – 20 and using good quality paper.
In the presence of your chosen witness, in black ink, sign each printed copy in both your old and new name
Have your witness enter their full name, address and signature on each printed copy
Use your signed and witnessed Deed Polls to inform your record holders of your legal name change.
*Please Note
You will need to fill in a new passport application to change your name on your passport and a DVLA application form to change your name on your driving licence.
